I hereby agree to adopt the above pet pig as a companion pet (NOT A FOOD SOURCE) and agree to Noah's Arks terms and conditions of adoption as more fully outlined………….

I understand that Noah's Ark has disclosed as much information as it knows about the pet pig...... however, no one can predict how a pet pig will react in a given situation especially without complete knowledge of it’s life experiences. Noah's Ark strongly encourages you to exercise caution and care in introducing your new pet into your home until he/she has become fully adjusted to the new environment and you have had the time to become familiar with your pet.

I understand the ownership and responsibility or the pet pig belongs solely to me. However, if for any reason the adoption does not work out to the advantage of the pet and myself I will immediately contact Noah’s Ark BEFORE FURTHER PLACING OR DISPOSING of the pet. I have paid total adoption fee of $_______________________and agree that these fees are non-refundable……….Noah’s Ark may at it's discretion exchange my pet for another animal within 30 days of the original adoption.

I agree that I will NEVER surrender or allow my pet pig to be GIVEN AWAY OR SOLD to an animal shelter, another rescue group, petting zoo, farm, new owners or otherwise disposed of WlTHOUT FIRST NOTIFYING NOAH'S ARK and allowing Noah’s Ark the option of reclaiming the pet. Should I fail to adhere to this provision of the Adoption Contract, Noah’s Ark may reclaim the pet from it’s new owners and I will be fully responsible to reimburse Noah’s Ark for any and all fees incurred to enforce this contract, including but not limited to legal fees, adoption fees and veterinary expenses.

I agree that my pet pig will never be allowed to run loose. I must have a fenced in yard and the pig must be in that yard unless I am walking that animal.

If I move to another location or relocate the home of my pet pig I will notify Noah's Ark as soon as possible.

If the pet pig is lost, stolen or passes away, I will notify Noah's Ark right away. If the pet is lost or stolen, I agree to use reasonable efforts to locate the pet including placing ads in at least two major newspapers and contacting my local animal control, animal shelter and sheriff dept. In case of death, I will provide Noah's Ark with any medical information including the cause of death, if known.

I agree to provide the best home possible to my pet pig, also to ensure an appropriate diet including pet pig food, water, shelter protection and any medical care needed during the pigs lifetime. I further understand Noah's Ark is under no obligation to pay for any expenses incurred for the pet once 1 have adopted it.

I agree to provide the pig with all appropriate veterinary care, including hoof trimming, tusk trimming (if applicable) and annual vaccinations (including at minimum) ERYSIPELAS and others recommended by my vet on an annual basis.

I understand Noah's Ark is not legally responsible for the behavior of this pet pig after adoption and I accept full legal and custodial and financial responsibility for the pigs actions. If the behavioura1 or medical problems arise I agree to seek appropriate humane assistance in correcting the problem so the pet can remain a member of my family.

I agree to allow Noah's Ark to consult follow up visits to my home and property.

I certify that I am legally able to have this pet pig at my home and have checked with the appropriate zoning officials in my township and /or county.

Noah's Ark reserves the right to revoke this adoption in order to protect the welfare o£ the pig. I understand and agree if the terms of this agreement are not upheld by the Adopter and/or if any misrepresentations have been made to Noah’s Ark in the adoption process, Noah’s Ark may terminate this agreement and reclaim the pig immediately, without notice from my home.

Should it become necessary for Noah's Ark to take Iegal action to recover an adopted pig or otherwise enforce the provisions of this agreement, the Adopter agrees to pay, all Court costs and attorney's fees. The venue for enforcement of this Agreement will be in Pasco County, Florida.

Noah's Ark makes no representation or warranties regarding the health, history, temperament or physical condition of any adoptive pet pig. All warranties including any implied warranties of merchantability and fitness for a particular purpose where applicable are expressly disclaimed.

Noah's Ark shall not be responsible for any bodily injuries or property damage caused by any adopted pet pig or by the actions of it's new owners. The adopter agrees to assume sole responsibility for the pet and agrees to hold Noah's Ark harmless from any and all loss or liabilities including all expenses by reason of any liability or claim threatened or imposed on Noah's Ark relating to the adoption of this pet pig or it's behavior or actions following the adoption.
